If you are an internal staff member who will be submitting requests on your own behalf (i.e. not for a group, team, club, etc), use your first and last name as the Group Name.

Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

Enter expiration date in MM/DD/YYYY format

If an organization is not a school-sponsored applicant, a Certificate of Insurance must be furnished prior to use of the facilities with the following minimum requirements: Amboy CUSD #272 named as an additional insured. Indication that coverage is provided for personal injury and damage to school property caused by the organization.  If the organization does not carry insurance, a waiver provided by the district must be signed by each participant's parent/guardian. This waiver then stays with the head of the organization in case anything would happen.
